COBRA Enrollment Information

This is a mandatory enrollment for Cigna healthcare. Effective January 1, 2021, there will be new Cigna Healthcare plans provided through the School Board. Therefore, in order to continue your participation in a School Board offered plan you must complete and return your 2021 Open Enrollment form in its entirety by November 20, 2020.

The Office of Risk and Benefits is not open to the public. For questions concerning the balance of your account or for enrollment assistance, please contact the Office of Risk and Benefits Management at 305-995-7137 or 305-995-1285, Monday – Friday, 8:00 a.m. – 4:30 p.m., ET.

Important Enrollment Information:

This is a mandatory enrollment for Cigna healthcare. Effective January 1, 2021, there will be new Cigna Healthcare plans offered: Cigna OAP High, OAP Standard and SureFit. Therefore, if you do not submit your completed enrollment form by November 20, 2020, the following will occur:

  • Your and your dependents’ current healthcare will be terminated on December 31, 2020 and you will not be allowed to re-enroll for benefits in the future.
  • Your current Flexible Benefits will continue for the 2021 plan year. 

There may be other coverage options available for you and your family and you may be eligible to purchase coverage through the Health Insurance Marketplace. In the Health Insurance Marketplace, you may be eligible for a new kind of tax credit that lowers your monthly premiums right away and you can see what your premiums, deductibles and out-of pocket expenses will be before you enroll. Being eligible for COBRA does not limit your eligibility for coverage for a tax credit through the Health Insurance Marketplace. Additionally, you may qualify for a special enrollment opportunity for another group or state-funded healthcare plan for which you may be eligible (such as a spouse’s plan). Healthcare Plans:

  • New Cigna plans are being offered: Cigna OAP High, Cigna OAP Standard, and Cigna SureFit*

 *Selection of a Primary Care Physician (PCP) is required at the time of enrollment and you must reside in the tri-county (Miami-Dade, Broward and Palm Beach) service area.

Flexible Benefits:

This enrollment is a changes only enrollment. The School Board continues to offer high-quality flexible benefits for eligible participants.

    • UnitedHealthcare Dental (DHMO & PPO)
  • Vision: EyeMed

2021 Benefits will not take effect if your 2020 account is not paid in full by the November 20, 2020 deadline. All premium payments must be made payable to: The School Board of Miami-Dade County, FL, and sent to: Office of Risk and Benefits Management, PO BOX 12241, MIAMI, FL 33101.

You will receive new billing coupons reflecting the premium for your benefits for the billing period of January 1, 2021 through December 31, 2021.

Covering Your Dependent(s):

If you are covering your dependent(s), you must indicate your dependent(s)’ Social Security Number in the dependent section of the enrollment form. You must also submit proof of eligibility documentation (i.e. marriage certificate for spouse, birth certificate for natural children, etc.).
