What You Need to Know:

Healthcare:

This is a mandatory enrollment for Cigna healthcare. Effective January 1, 2021, there will be new Cigna Healthcare plans offered: Cigna OAP High, OAP Standard and SureFit*. Therefore, if you do not make a healthcare election during the enrollment period, the following will occur:

  • Your and your dependent’s current Cigna healthcare coverage will terminate on December 31, 2020.

*The Cigna SureFit requires the selection of a Primary Care Physician (PCP). If a PCP is not selected, Cigna will assign you a participating provider based on your zip code. You must live in the tri-county (Miami-Dade, Broward and Palm Beach) service area.

To obtain a healthcare benefits enrollment package, contact the Office of Risk and Benefits Management at 305-995-2883, Monday – Friday, 8:00 a.m. – 4:30 p.m.

NOTE: There is no Board contribution towards the employee’s and dependents’ coverage. Part-time employees are responsible for the full monthly cost of the Board-approved healthcare plans. The cost of these benefits will not be deducted from your paycheck.

Flexible Benefits:

This is a changes only enrollment. If you are currently enrolled in Flexible Benefits and you would like to continue your benefits for the 2021 plan year, you DO NOT need to return an enrollment form. Your current coverage and your dependent(s)' coverage will continue. The cost of these benefits WILL NOT be deducted from your paycheck. FBMC Benefits Management will send a monthly invoice for your flexible benefits.

If you wish to make changes to your current benefits or enroll for benefits effective January 1, 2021, you will need to complete and submit an enrollment with your first monthly premium payment. To obtain an enrollment form, please contact FBMC Benefits Management, Inc., at 1.855.MDC.PS4U (1.855.632.7748), Monday – Friday, 7:00 a.m. – 7:00 p.m., ET.

Dependent Coverage:

  • You can add your eligible dependent during this enrollment period and you must provide a valid social security number for each dependent. Additionally, you will have to submit dependent eligibility documentation upon request (i.e., marriage certificate for spouse, birth certificate for natural children) for all covered dependents. If you do not provide the required documentation, coverage will be terminated.
  • You can terminate your dependent coverage, but you will not be able to re-enroll the dependent until the next Open Enrollment provided you have maintained your enrollment in a School Board sponsored healthcare plan. You can add a dependent throughout the year if experiencing a qualifying change in status event.
  • You and your dependent must be enrolled in the same healthcare plan.

Termination of Employment:

If you are no longer employed by the School Board, you will be provided the opportunity to continue your benefits in accordance with the Federal COBRA Law. You will be provided the ability to continue your medical, dental and/or vision plan if active and paid up to date at the time of termination.
