AFSCME Part-time Food Service Employee Benefits

This is a mandatory enrollment. Effective January 1, 2021, there will be new Cigna Healthcare plans provided through the School Board; therefore, your current healthcare coverage will terminate on December 31, 2020.

A Benefits Representative is ready to assist you with your enrollment.  The Office of Risk and Benefits is not open to the public; however, you can call FBMC Benefits Management at 305-995-1799, Monday – Friday, 8:00 a.m. – 4:30 p.m. for assistance.

What You Need to Know:

Important Enrollment Information:

This is a mandatory enrollment for Cigna healthcare. Effective January 1, 2021, there will be new Cigna Healthcare plans offered: Cigna OAP High, OAP Standard and SureFit*. Therefore, if you do not make a healthcare election during the enrollment period, the following will occur:

  • Your and your dependent’s current Cigna healthcare coverage will terminate on December 31, 2020.   
  • If you are currently declining healthcare coverage, your opt-out election will terminate on December 31, 2020.
  • If you are being deducted the spouse/domestic partner annual surcharge, the deductions will terminate December 31, 2020.
  • You will be automatically auto-assigned to the Cigna SureFit* healthcare plan, which will be the free Board-paid option for all benefits eligible employees.
  • Any employee who covers a spouse or domestic partner will be required to disclose their dependent’s access to an employer sponsored healthcare plan. Employees covering a spouse or domestic partner may be required to pay an additional surcharge.
  • If you are currently enrolled in Flexible Benefits (i.e. dental, vision, HIC, legal, etc.), those benefits will continue for the 2021 plan year. If wish to make changes or enroll, please complete and return the 2021 Flexible Benefits Enrollment form by November 20, 2020.

 

*The Cigna SureFit requires the selection of a Primary Care Physician (PCP). If a PCP is not selected, Cigna will assign you a participating provider based on your zip code. You must live in the tri-county (Miami-Dade, Broward and Palm Beach) service area.

Healthcare Plans:

New Cigna plans are being offered for the 2021 plan year: Cigna OAP High, Cigna OAP Standard, and Cigna SureFit.

  • The Cigna SureFit plan will be the free option for all benefits eligible employees.

Dependent Coverage:

  • If you are covering your dependent, you must indicate your dependent’s social security number and provide documentation of proof of eligibility upon request. If you do not provide the required documentation, coverage will be terminated.
  • You can terminate your dependent’s coverage, but you will not be able to re-enroll the dependent until the next Open Enrollment. You can add a dependent throughout the year if experiencing a qualifying change in status event.
  • You and your dependent must be enrolled in the same healthcare plan.
  • If you cover your spouse/domestic partner on your healthcare plan and your spouse/domestic partner has coverage available from his/her own employer, an additional annual surcharge of $500 will be charged. The annual surcharge will be billed on a biweekly basis according to your pay schedule. If your spouse/domestic partner does not have an employer sponsored healthcare plan available to him/her, please make sure to select the appropriate response on the 2021 Healthcare Benefits Open Enrollment Form and the spousal/domestic surcharge will not be applied.

Termination of Employment:

  • If you are no longer employed by the School Board, you will be provided the opportunity to continue your benefits in accordance with the Federal COBRA Law. You will be provided the ability to continue your medical, dental and/or vision plan if active and paid up to date at the time of termination.
  • Should employment terminate during the last month of the school year, coverage will cease at the end of the calendar month in which your employment terminates. Benefits will remain in effect through August 31st.
